Manchester Mental Health Trust Sex Offender

An unnamed bearded male who identifies as a transgender woman was reported in The Times to have repeatedly exposed himself to female patients in the care of Greater Manchester Mental Health NHS Foundation Trust.

The allegation came to light after a consultant psychiatrist, anonymised as “Dr A”, lodged a tribunal claim against Greater Manchester Mental Health NHS Foundation Trust in August 2026. The Times reports that after patients complained about the behaviour and presence of the male patient who identifies as “female”, he was moved to a male ward. However, managers at the trust intervened and the man was returned to the female ward. Dr A points out that many of the women on the female ward were detained under the Mental Health Act 1983 and included victims of sexual violence – i.e. they were extremely vulnerable, potentially unable to advocate for themselves and certainly unable to escape a male patient who was an undoubted threat to them.

Dr A points to the trust’s “Delivering Single Sex Accommodation” policy as the root of the problem, as it (wrongly) states that trans patients should be accommodated according to their presentation, including for toilet and bathing facilities. Her claim is based on her expectation that the policy should be amended so it complied with the 2025 Supreme Court ruling that “woman” and “man” in the Equality Act 2010 referred to biological sex – i.e. a single sex ward for women cannot admit any male patients, whether or not they identify as transgender. Dr A’s repeated requests for the trust to clarify its plans to change the policy were met with delays, resistance and suggestions that the judgment might be challenged in the European Court of Human Rights – as if a wholly theoretical court challenge should override the actual law in the UK.

In June 2025, Dr A lodged a formal grievance over the trust’s alleged failure to implement the ruling,  which was partially upheld. She alleges that 16 months after the Supreme Court ruling, the Greater Manchester Mental Health trust has neither amended its key policies nor stopped admitting biologically male patients to female psychiatric wards, and identifies this as a serious safeguarding risk. Her tribunal claim, filed in August 2026, alleges that the trust’s response to her concerns was unnecessarily prolonged and distressing, and amounted to unlawful discrimination because of her protected beliefs.

Damian Hinds, the Shadow Secretary of State for Health and Social Security and Joy Morrissey, Shadow Minister for Women & Equalities have written to the Chief Executive of the Greater Manchester Mental Health NHS Foundation Trust to demand an explanation and seek to restore safeguarding for women.

A recent report in the Daily Telegraph highlighted how the NHS is still ‘dragging its feet’ on single-sex space rules, meaning women are still being put at risk from men who identify as transgender.

  • In 2026, a violent trans-identified male named Zack/Kalani Jay was convicted of trying to suffocate a female patient in a Cornwall psychiatric unit. He had previously tried to suffocate his grandmother and was also convicted of threatening to kill his male partner.
  • In 2018, a female patient who was acutely unwell in a locked Lancaster psychiatric unit, raised her concerns with hospital staff about a trans-identified male housed with her, but her fears were dismissed and her medical notes implied that she was a “transphobic bigot”.
  • Reported earlier in 2026, a complete failure in safeguarding occurred within Bethlem Royal Hospital in south-east London; a female patient who identified as a transgender man was raped within an hour of being placed in a male psychiatric ward.
  • In 2021, an unnamed male who identifies as transgender is alleged to have raped a female patient in an NHS hospital. The allegation came to light during a House of Lords debate in March 2022 on single-sex wards in hospitals. Peers heard that following a complaint of rape, hospital staff advised police that that “the rape could not have happened” because no male was present. 
  • In the UK in 2022, a hospital cancelled a woman’s operation and accused her of discrimination against a trans-identified male nurse when she objected to him being involved with her care – she had stipulated needing female-only care. The hospital has since apologised, admitting that what happened was a breach of the patient’s privacy.
  • In Germany in 2022, a 40 year-old woman named Kamilla Nagy was viciously murdered by a violent trans-identified male patient in a psychiatric unit near Munich. He admitted torturing Ms Nagy with a metal bar, strangling her with her sweater, and then setting her on fire, but her family’s calls for a proper investigation have been blocked by authorities.
